ORDERING INFORMATION
CREDIT CARD: We accept Paypal, MasterCard, Visa, American Express, and Discover credit cards for order payment. Our charge will appear on your credit card statement as "Low Vision Mart"
SALES TAX: We must charge sales tax on all orders within the
state of Nebraska (NE) and Iowa (IA). No sales tax will be charged to orders from other locations, or tax exempt organizations. (Please provide tax exempt certificate with order.)
ORDERING BY FAX: Make a copy and complete the order form found at Order Form, and fax it to 402-491-3221. Make sure that you have included your credit card number. (There is no need to include a cover page with your fax.)
ORDERING BY MAIL: If you prefer to mail us your order please include a copy of the Order Form, and mail it along with your credit card number, money order, or personal check to:
ORDERING BY PURCHASE ORDER: Purchase orders are accepted from federal and state government, educational institutions, and approved corporations.
$7.00 if the total order is under $40.00
$10.00 if the total order is $40.01 to $100.00
$14.00 if the total order is $100.01 to $150.00
$20.00 if the total order is $150.01 to $200.00
$25.00 if the total order is over $200.00
